Appointments

For your convenience, our office is open daily Monday ​through Friday.


To schedule an appointment, please call 781-449-​3588 for appointments at our Needham office. ​Registration and medical history forms may be ​downloaded from this website. Please fill them out ​and mail them back or bring them with you. At your ​first appointment, please arrive 15 minutes early to ​complete any additional paperwork.


To cancel or reschedule an appointment, please give ​us 24 hours notice.


If you are consulting one of our physicians about a ​skin rash, please bring a list of your current ​medications including prescription and ​nonprescription items. If possible, bring the actual ​packaging or containers of any topical medications ​you are using. They often contain a listing of ​ingredients which may help us to make a diagnosis.

Emergencies

We reserve several appointments ​each day for emergencies. In case ​of an emergency after business ​hours, Dr. Cohen can be reached ​by a telephone paging system.

Insurance

If your insurance requires a referral for office visits to a specialist, we ask ​that you contact your primary care physician's office prior to your visit, so ​that your referral will be in our office on the date of your appointment. If ​we do not have a referral you will be asked to sign a waiver, as required by ​your insurance company.


We participate in most managed health care plans. Our billing service will ​be happy to submit claims to your insurance company for you. However, ​please remember that your insurance policy is a contract between you ​and your insurer. All insurance copayments, deductibles, and payments ​for non-covered services such as cosmetic procedures are due at the time ​of service, unless other arrangements are made in advance.
